That more than 15 days without internet ... hard ...
Still alive ... despite many vicissitudes. Starting by Bassila. Tuesday morning as planned we leave for Bassila. On our arrival, we are very well received by the storekeeper. He asks us to call the depot manager who was on vacation for him to confirm our work. Very nicely, Sabrina handed me the phone because for the few conversations she had with this gentleman and explanations of the warehouse, we expect to have someone on the phone a bit unpleasant. So I call upon the dear gentleman ... and there is the drama, he spent 20 minutes yelling on the phone ... like what he did not want us to work at his deposition absence alors que nous l’avions prévenu de notre arrivée. J’ai donc été obligée d’appeler son supérieur hiérarchique pour que le gestionnaire nous laisse travailler. Il faut savoir qu’apparemment ce monsieur est désagréable avec tout le monde et qu’il a failli en venir aux mains avec un de ces collègues. Aurait-il des choses à cacher ? Nous y retournons donc le lundi 21 juin pour le rencontrer (300km dans la journée !) !!! Même pas peur d’abord ! A part ça, le magasin du dépôt est juste parfait : Climatisé, propre, bien rangé, pas de carton sur le sol, fiche de stock à jour !!! Coté informatique ils utilisent Médistock. in SAVE, the work went well. No special adventures for a change. Team deposit invited us to eat pounded yam with sesame sauce because it is the region. Really delicious! A highly available and ready to do anything to help us. By cons, their working conditions are not easy. Indeed, the deposit (built by the state) is located at the area hospital while staff is responsible for filing at the box office which is 3 km from the hospital. In addition, there no staff dedicated specifically to the filing: the warehouse is committed to the SCA, the manager is accountable for the Area Office. There are so many problems in the overall management of the deposit. The coefficient of sales for the deposit is 1.1 (as for all DZ in this region). The deposit is not financially independent. All expenses are borne by the health zone. They use software Médistock as medication management. We were able to discuss with a person who provided the training on this software. Very informative. The software is generally tailored to the needs of the DZ. There remains some inconsistencies particularly in the calculation of the deposit needs to be done in months, not quantity. Moreover, prices are set by the computer (after setting the multiplier when setting) and therefore it is impossible to decreasing margins smoothed high prices (if you want to change the price of a product must do so at the time of billing every time!! Not Easy, is one reason that prevents most of DZ using this software for drugs other than CTA). The software allows to know the needs of health facilities (as PharMeg) DZ but finds himself facing the same problem Borgou-Alibori where health facilities do not provide the information necessary to calculate. The great strength of the software is having a maintenance Benin which moves in case of problems, and conducts supervision every 3 months with retraining. We sent our detailed comments to the president ... we expect his remarks. In addition, we hope to meet this week PISAF team to learn more about the software and their various actions at the DZ. You should know that they have funded the creation of a procedures manual for management of health zones in which a portion is devoted to DZ. This manual is being validated and should be released in January 2011. We were able to get the manual (we're good detectives ...) that seems very complicated ... to see!
We learned that after reshuffle at the Ministry of Health, no longer exists. Confirmation from Dr. Adonon we got on the phone. He is currently on stand-by. Still, the Health Zone and therefore the DZ have more direction at the central level. So we have more referent to the Department of Health. We are told that those responsible are now the Health Zone DDS (Departmental Directorate of Health). But this does not facilitate a harmonization action of DZ. On our return to Cotonou, we absolutely need to work again with the authorities of the department to create new contacts for PSF37.
Otherwise, next to the work, Geraldine wanted to show me his village of heart Kokoro for the weekend. She warned me that it would not be comfortable! We ended up sleeping in a room of motherhood that looks more like a closet than a room when it is 2. We still managed to cram our many bags. Despite these minor inconveniences, the weekend was really nice to Kokoro. We saw the nursery while Kokoro, all friends of Geraldine! It was full of peanuts, was offered a bottle of honey Ge home, we ate grout grout just too good. We lived in the village all its reality and we are divided memories Full head and heavy heart.
We arrived at Savalou a bit tired and we put much hope in the hotel but were disappointed: Hotel very clean (for a high price) and extremely noisy. A cook who wanted us to pay 2500 CFA spaghetti! Not really ideal for a dépôt.Nous have never seen a deposit with as many breaks. Over a year, almost 100% of drugs observed had at least one fracture. And the worst is that many of these drugs had more than 3 fractures (breaks and long ...). The satisfaction rate of health suffers automatically. It is here only 58%! According to the storekeeper would be the fault of the CAM. But we can not verify it because it does not keep track of his orders to the CAM. The deposit is not financially autonomous but aims to become one. In addition, the RAC would like there to be a depot manager for the warehouse is currently designated as the depot manager, but lacks the management skills needed (niveau scolaire 3éme sans le Brevet) et le comptable du bureau de zone qui est sensé le seconder est un peu perdu. Ils devraient utiliser Médistock mais le comptable ne maitrise pas le logiciel et le magasinier n’a pas accès à l’ordinateur. La personne qui a reçu la formation avec le magasinier est partie sur Savé.
Nous avons évolué à Dassa pour le week-end dans un hôtel très calme qui possède une autruche hermaphrodite (nous, on a toujours pas compris ?!).
Après un week-end de repos, de lavage et de travail, nous sommes prêtes à attaquer une nouvelle semaine qui commence demain par la rencontre du gestionnaire aimable de Bassila et en fin de week meeting with PISAF. We leave at Abomey Tuesday. We attack our last 4 DZ. Hopefully the logistics will be easier from now on ...

Thursday, May 27, we left for Tanguieta Natitingou. Natitingou work was somewhat disturbed by the unexpected visit of the NMCP (National Programme for the Fight against Malaria) which has totally monopolized the staff as to Tanguieta only worse. To believe they are following us ... For lodging, we were able to retrieve information but not necessarily in the best circumstances. We had to redo our program every day and see every hour of the day. Not easy to remain effective. But the work was done! In summary, the current deposit was built in 2008 according to plans by the state but with a layout (shelves) much more functional than in the Borgou-Alibori, where all space is occupied. The deposit is like Tanguieta, highly integrated office area but with a coefficient of drug sales more reasonable 1.2. It is still important to you to note that this deposit is near (within 10 m) of the CAM. It is difficult to understand what is relevant to health facilities to go to the deposit rather than CAM. Especially that the calculation of satisfaction of health facilities at the depot is only 64%! But the filing date of 1994 and therefore predates the establishment of the CAM Natitingou. The area has therefore developed with the financial support of the deposit. Computer side, the staff consists of a clerk (CM2 level) and one count filing that uses the software Médistock for the management of malaria commodities. Management of other drugs is done with Excel. There is therefore no real computer management of medicines.
All right, this led us to Friday night, end of our workweek. We dined at the Breach of Nati, a small restaurant in a Tata Somba (traditional dwelling of the region) and enjoying a superb view of the mountains and Natitingou. Saturday morning stroll to town. Then move to the market for tomato salad and tuna-lawyer-banana for dessert. At the moment we eat mangoes Morning and Evening as they are just delicious! We ended our day with la visite du musée de Nati où sont expliqués les us et coutumes de la région. Très intéressant surtout en vue de notre balade du dimanche… La nuit de samedi à dimanche fut, comment dire, légèrement bruyante : en gros, on n’a pas dormi de la nuit pour cause de rites funéraires qui se traduisent par le bruit du tam-tam toute la nuit juste derrière le mur de notre chambre! Un peu dur pour des Yovos mais ça nous a permis d’aller les voir le lendemain matin et de découvrir en quoi consistent les rites funéraires ici. Puis nous sommes parties pour plus de 100 kms de piste à moto à la rencontre des Tatas Somba avec un très bon guide nommé Baba. La journée s’est terminée par la visite des chutes de Kota (pour y accéder, nous avons fait du moto-cross et de l’escalade !). Très bonne journée où on s’en est pris encore plein les yeux…que de souvenirs !!!
Un bon week-end pour démarrer une semaine intense de travail. Lundi, dernier jour à Natitingou.
Mardi départ pour Djougou. Nous avons été accueillies par le CAR. Nous avons travaillé avec l’équipe de Djougou toute la semaine. Cette équipe un peu atypique (Un comptable comme gestionnaire, un commis/magasinier et une opératrice de saisie (présente que pour taper les commandes des formations sanitaires sur l’ordinateur) a vraiment été très disponible. Ce fut un réel plaisir de travailler avec cette équipe dynamique et très sympathique. Nous avons eu toutes les informations nécessaires pour notre étude. Ici aussi le Dépôt est très intégré au bureau de zone. Suite à des détournements de médicaments, le dépôt a connu des difficultés financières nécessitant la prise en charge de ses dépenses par le bureau de zone jusqu’en décembre 2009. L’objectif du CAR est qu’à terme le dépôt devienne autonome. Le dépôt est vraiment très bien rangé. Il utilise un coefficient de prix de vente des médicaments de 1,1. Le dépôt n’utilise aucun logiciel medication management. Despite Medistock training received, the software does not give satisfaction to the team. There is work to be PSF37 especially as the "motel lake" Djougou is just a small haven of peace (despite the number of mosquitoes that love Yovos). We spent a weekend very studious. We enjoyed the quietness of the hotel to start comparing the different data collected in the first 5 deposits. Good for 6th

Finally, the deposit is far from all other places of our investigation, it is difficult to make return trips. You think that's it ... it would be too easy. We had to reverse and Savé Savalou in the order of our visits because staff is supervised during Savalou 15 days. Not easy to catch these Beninese ... one wonders how do the health facilities when they need medication. In all cases we remain focused and hopefully come to the end of the study within the allotted time.
PS: RAC Djougou wanted to introduce ourselves to the director of the hospital area. How you say it was a bit violent. We'll explain. This hospital is run by the Order of the Cross of Malta well known people who have followed the debates about MNU. Indeed, they were very vocal against the decision to stop MNU because it was their stock in trade. In short, we found that virulence to the director of the hospital (one French) who has literally assaulted when we arrived in his office. These are people that no discussion is possible. We still tried to defend the position of PFS ...
RAC was a little embarrassed by this altercation (where we stayed calm and got excited when the director himself!). But we explained the reason for this debate that ignited the French associations for some time.
In conclusion, ENJOY THE END OF MNU !!!!!
