Still a lot of adventures for our last 3 deposits.

Abomey We left Friday, July 2 in the morning to go to Lokossa. Normally work at the depot but it would be too easy, although we had a little challenge for the end of our study. Indeed, we learned that the depot manager was on leave for 1 month ... hard to make an inventory of a deposit when the main staff is absent. We therefore had to find another deposit in a week ... Not easy when you do not know any team in the south. But we were very lucky because Dr. Tokpo Simplice (former coordinator of Nikki who is now the Ministry of Health program neglected diseases) has made contact with his fellow physicians and coordinators of the south we found a small deposit. We changed to Lokossa Avrankou was our 12th and last deposit. So
Friday, July 2, we met with the DDS and the pharmacist SMC Mono-Couffo. Always found interesting when you see the little information they have on deposit. The pharmacist SMC agrees that it lacks a true pharmaceutical expertise in deposits. But there are too few pharmacists in the public sector in Benin to address this problem. The partners could have an interesting role to play at this level (training and monitoring teams, ...). But it is also necessary that there are pharmacists who work in their teams! Ceci peut s’expliquer par le fait qu’il n’y a AUCUN pharmacien dans leurs équipes… Alors qu’ils travaillent sur la chaîne du médicament…ABERRANT et ENERVANT !

Bon nous avons donc dormi à Cotonou le vendredi soir pour nous rapprocher de Porto-Novo, notre destination de la semaine suivante. Nous avons retrouvé l’effervescence, la pollution, le bruit de la ville de façon un peu brutale… ("What we're well north!")
After the match Ghana-Uruguay (we had two match: one in the TV, the other between 2 ... Benin), we were invited to go see a concert Tiken Jah (Reggae, International). Concert was to start at 20h, where we arrived at 22:30 ... which had not yet begun ... We are waiting ... until ... 1:30 in the morning to see the beginning of the concert organizers had not paid musicians and others in the evening ... Et c’est vraiment énervant de voir comment ils n’assument pas leurs responsabilités…c’est toujours la faute des autres ! Comme toujours dans le sud, le dépôt n’a aucune autonomie financière vis-à-vis de la zone. Il est donc très difficile pour nous d’évaluer les dépenses réelles du dépôt pour évaluer le coût operation. Hence the importance of monitoring of expenditure by the office area! This deposit made no margin on the sale of drugs until this year. They are now applying a factor of 1.1 but without any justification, just because they saw it was like that elsewhere ... feedback to central health authorities and the CAM. But not easy to catch them now: vacation, supervision, training, meeting ... but we hope to see Thursday and Friday. Bon que nous est-il arrivé ces derniers jours outre l’inondation et la voiture qui apprend à nager.
Nous sommes donc arrivées mardi matin à Abomey. L’équipe était là. Enfin le gestionnaire nous dit qu’il est libre qu’une journée car après il va former les gens de la zone sur le logiciel Médistock (j’ai travaillé en fin de journée avec lui sur le logiciel et bien j’en connaissais plus que lui et il était incapable de répondre à la moindre question un peu pharmaceutique : comment le logiciel calcule les paramètres de gestion pharmaceutique… c’est grave quand même quand on pense que c’est un formateur…). Well we must go faster ... We continue to work the following days without the manager. A little hard to get all the documents we want, but after four days we're getting there!! The depot building is a classic built by the state. This deposit is "helped" by a program that buys drugs they sell and where they deposit money into a special account for future hospital zone. Sunday we took a guide for visiting artists Abomey (traditional and contemporary) as well as learn about the history of the Amazons. Well the visit began somewhat laboriously. 1hr to tell the story of Abomey we already knew, without story and a way quite soft and very very slow ... I thought it was Sab partir en courant…

Lundi lever à 5h45 pour Sab (moi j’ai pu grappiller 30min de sommeil en plus) car nous voulons être à 8h30 à Aplahoué et selon Eric nous avons 1h30 de route…donc départ à 7h avec p’tt dej à l’hôtel to 6h30.Bref we leave ... and 43min later we arrived ... Uh ... there's a problem of estimating the travel time, is not it?? When we ask the evening to Eric why he thought that the trip lasted 1:30, he tells us "ben I purposely say 1:30 instead of 45 min .." uh ... here we do not understand the joke ...

A part ça, le magasin du dépôt est juste parfait : Climatisé, propre, bien rangé, pas de carton sur le sol, fiche de stock à jour !!! Coté informatique ils utilisent Médistock. During our last night we made "attack" by mice. So we bravely fled and we slept under the porch of motherhood on cement benches, with bats above our heads. Having woken Rosalie (the carer) by our move, she wanted to sleep with us. So we slept with bats, mosquitoes and other critters ... and snoring Rosalie! A little hard! Despite these minor inconveniences, the weekend was really nice to Kokoro. We saw the nursery while Kokoro, all friends of Geraldine! It was full of peanuts, was offered a bottle of honey Ge home, we ate grout grout just too good. We lived in the village all its reality and we are divided memories Full head and heavy heart.

Après un week-end de repos, de lavage et de travail, nous sommes prêtes à attaquer une nouvelle semaine qui commence demain par la rencontre du gestionnaire aimable de Bassila et en fin de week meeting with PISAF. We leave at Abomey Tuesday. We attack our last 4 DZ. Hopefully the logistics will be easier from now on ...

Thursday, May 27, we left for Tanguieta Natitingou. Natitingou work was somewhat disturbed by the unexpected visit of the NMCP (National Programme for the Fight against Malaria) which has totally monopolized the staff as to Tanguieta only worse. To believe they are following us ... For lodging, we were able to retrieve information but not necessarily in the best circumstances. We had to redo our program every day and see every hour of the day. Not easy to remain effective. But the work was done! In summary, the current deposit was built in 2008 according to plans by the state but with a layout (shelves) much more functional than in the Borgou-Alibori, where all space is occupied. The deposit is like Tanguieta, highly integrated office area but with a coefficient of drug sales more reasonable 1.2. It is still important to you to note that this deposit is near (within 10 m) of the CAM. It is difficult to understand what is relevant to health facilities to go to the deposit rather than CAM. Especially that the calculation of satisfaction of health facilities at the depot is only 64%! But the filing date of 1994 and therefore predates the establishment of the CAM Natitingou. The area has therefore developed with the financial support of the deposit. All right, this led us to Friday night, end of our workweek. We dined at the Breach of Nati, a small restaurant in a Tata Somba (traditional dwelling of the region) and enjoying a superb view of the mountains and Natitingou. Saturday morning stroll to town. Then move to the market for tomato salad and tuna-lawyer-banana for dessert. At the moment we eat mangoes Morning and Evening as they are just delicious! We ended our day with la visite du musée de Nati où sont expliqués les us et coutumes de la région. Très intéressant surtout en vue de notre balade du dimanche… La nuit de samedi à dimanche fut, comment dire, légèrement bruyante : en gros, on n’a pas dormi de la nuit pour cause de rites funéraires qui se traduisent par le bruit du tam-tam toute la nuit juste derrière le mur de notre chambre! Un peu dur pour des Yovos mais ça nous a permis d’aller les voir le lendemain matin et de découvrir en quoi consistent les rites funéraires ici. Puis nous sommes parties pour plus de 100 kms de piste à moto à la rencontre des Tatas Somba avec un très bon guide nommé Baba. La journée s’est terminée par la visite des chutes de Kota (pour y accéder, nous avons fait du moto-cross et de l’escalade !). Très bonne journée où on s’en est pris encore plein les yeux…que de souvenirs !!!

Un bon week-end pour démarrer une semaine intense de travail. Lundi, dernier jour à Natitingou.
Mardi départ pour Djougou. Nous avons été accueillies par le CAR. Nous avons travaillé avec l’équipe de Djougou toute la semaine. Cette équipe un peu atypique (Un comptable comme gestionnaire, un commis/magasinier et une opératrice de saisie (présente que pour taper les commandes des formations sanitaires sur l’ordinateur) a vraiment été très disponible. Ce fut un réel plaisir de travailler avec cette équipe dynamique et très sympathique. Nous avons eu toutes les informations nécessaires pour notre étude. Ici aussi le Dépôt est très intégré au bureau de zone. Suite à des détournements de médicaments, le dépôt a connu des difficultés financières nécessitant la prise en charge de ses dépenses par le bureau de zone jusqu’en décembre 2009. L’objectif du CAR est qu’à terme le dépôt devienne autonome. Le dépôt est vraiment très bien rangé. Il utilise un coefficient de prix de vente des médicaments de 1,1. Le dépôt n’utilise aucun logiciel medication management. Despite Medistock training received, the software does not give satisfaction to the team. PS: RAC Djougou wanted to introduce ourselves to the director of the hospital area. How you say it was a bit violent. We'll explain. This hospital is run by the Order of the Cross of Malta well known people who have followed the debates about MNU. Indeed, they were very vocal against the decision to stop MNU because it was their stock in trade.
